Consent Form

I hereby provide my consent and acknowledge the following terms and conditions regarding the permanent jewelry procedure to be performed by Linked by Ruthie. Check box to confirm consent.

The procedure involves the non-invasive application of permanent jewelry (such as bracelets, anklets, or necklaces), which is welded together in place of a traditional clasp. This procedure will be performed by a trained and experienced technician.

I understand that while the procedure is generally safe, there are inherent risks including, but not limited to:
1. Skin irritation, allergic reactions, or infection at the site of application.
2. Discomfort during or after the procedure.
3. Risk of the jewelry becoming tight or uncomfortable over time.
4. Potential for the jewelry to catch, break, or cause injury if not properly cared for.

I confirm that I do not have any known allergies to metals or materials used in the jewelry, and that I have disclosed any relevant medical conditions (e.g., skin sensitivities, autoimmune diseases, or history of infections). I will inform the technician of any changes in my medical condition before receiving the procedure.

1. Avoid excessive use of perfumes, lotions and chemicals.
2. Clean your permanent jewelry with mild, non-abrasive soap and warm water.
3. Avoid tugging, twisting, or applying too much pressure to your jewelry, as it can weaken.
4. To preserve the quality and appearance of your jewelry, avoid exposure to chlorine, which may cause discoloration or weakening of the metal. If contact occurs, gently clean the piece with mild soap and water. Please note that we are not liable for any damage or discoloration resulting from chlorine exposure.
5. Sterling silver may naturally tarnish over time. To maintain its shine and prevent discoloration, we recommend cleaning it periodically with a jewelry polishing cloth.
6. We will always re-weld a broken piece if it breaks at the jump ring.
7. We are not responsible for misplaced or broken chains.
8. To remove your jewelry, carefully cut the jump ring—the small circular ring used to weld your chain—using scissors or toenail clippers.
9. I agree to follow the aftercare instructions provided by Linked by Ruthie to minimize risks of irritation, infection, and other complications.

I acknowledge that proper maintenance, including regular cleaning and tightening of the jewelry, may be required to preserve its condition. I understand that neglecting maintenance may reduce the lifespan of the jewelry.

I hereby give permission to Linked by Ruthie for any photos, videos, or audio that are taken of me during the permanent jewelry process to be used in and/or for any lawful promotional materials. These materials may include, but are not limited to, newsletters, flyers, posters, brochures, advertisements, press kits, websites, social media pages, and other print and digital communications related to the promotion and marketing of the Jewelry Business. This authorization shall continue indefinitely and extends to all languages, media, formats, and markets now known or later discovered. I renounce all claims I may have to royalties or other forms of payment resulting from or connected to the use of the image, video, audio, or any other recording. I understand and agree that these materials shall become the property of the Jewelry Business and will not be returned.

I acknowledge that the practitioner has thoroughly explained the procedure, risks, and aftercare instructions to me. I have had the opportunity to ask any questions and have received satisfactory answers.
